Develops and manages Home Care Liaisons, new referral business relationships and loyalty to fulfill new business growth, goals and objectives. Facilitates service to meet the needs of the referral source. Promotes Home Care Network clinical programs and services to physician and community resulting in an increase to referrals and home care admissions.
Participates in the development of long/short term strategic plans in conjunction with Network leadership.
Develops relationships between physicians, system case management, community sources, organizations, and sub-acute facilities to promote Home Care Network programs/services.
Visits physician offices and rehab facilities, to update, promote and apprise physicians of Home Care Network initiatives and services to increase referrals and awareness of the Home Care Network.
Analyzes and identifies physician needs, offers appropriate solutions and provides timely follow-up to physician inquiries relating to Home Care Network services.
Manages and coordinates the Home Care Liaison staff and their activities.
Manages, hires, trains, disciplines and evaluates the performance of staff in collaboration with clinical management. Ensures performance appraisals are completed in a timely manner.
Collaborates with management concerning personnel problems and any subsequent disciplinary actions.
Oversees and arranges the scheduling/staffing of Home Care Liaisons at sub acute facilities.
Participates in the on-going functioning, growth and development of the Home Care Network.
Develops and updates customer database for ongoing Home Care Network opportunities.
Obtains data collection and analysis for productivity measurement and analysis of referral/admissions. Prepares monthly reports on referral/admission statistics.
Reports market intelligence, identifies issues considered barriers to referrals and recommends solutions.
Identifies potential shifts in service care needs or health care trends impacting future planning. Promotes awareness of the Home Care Network and its services within the Health System and to the general community.
Participates and/or leads the promotion of community education programs with the Home Care Network.
Coordinates development of marketing plans and materials to attract new referral sources.
Coordinates and presents Home Care presentations and information to patients at pre surgical TJR classes throughout the system.
Develops and implements high standards of quality care.
Participates in performance improvement activities.
Reports the outcome of evaluation activities of staff.
Schedules regular in-service programs for professionals to enhance their knowledge base.
Provides information to staff on relevant continuing education opportunities.
Interprets the Home Care Network’s policies and procedures.
Serves as a participating member on the Network’s professional committees.
Takes problem/complaint calls, when necessary, from patients, families, physicians and/or hospitals.
Collaborates with managers and staff of all home care agencies of the Health System Home Care Network.
Performs job functions in a fiscally responsible manner consistent with the Network’s mission and vision.
Performs related duties, as required.
Bachelor’s Degree in Business Administration, Healthcare Administration or related field, required. Master’s Degree, preferred.
Valid New York State Driver’s License, required.
Minimum of five (5) years related healthcare business development experience, required.
Proficient in Microsoft Word, Excel and Powerpoint, required.
Excellent written and oral communication skills, required
Licensed Registered Nurse in the state of New York, highly preferred.
