To perform a variety of duties to ensure that patients and visitors are provided with services that reflect the hospital commitment to a safe, secure and customer focused environment. To assist in patient care related activities of daily living and focusing on the patient experience. To assist in meeting meaningful use requirements as it relates to access to the patient portal.
Maintains a safe work environment by following Safety and Infection control guidelines and adopts safe work practices.
Acts as the liaison between patients, their families/friends, and the hospital staff in an effort to keep the lines of communication open for all concerned parties.
Promotes positive customer service by effectively interacting with peers, managers, medical staff, patients and guests with care, compassion and respect
Independently initiates patient contact. Makes patients rounds in designate areas.
Provides care that reflects an understanding of individual cultural and religious diversities and needs, as well as recognition of patient rights.
Makes everyone feel welcome by smiling and introducing his/herself to patients and visitors. Always responds with care and compassion
Support Care Associates observe the patient and maintain a safe environment, and as delegated by the nurse.
Takes the extra time to listen and carefully to show patients, guests, and coworkers that their concerns, questions, and comments are important and take appropriate action.
Ensures supplies and equipment available and in working order according to established department protocols. Replenishes supplies in patient rooms, fills water pitches
Responds to call lights outside patients’ rooms; notifies staff as needed.
The Support Care Associate is responsible for ensuring that the patient’s dignity is preserved.
Acts as an advocate for promoting safety, such as fall prevention, hand washing, isolation protocol and maintaining an orderly environment.
Performs other duties as assigned or required.
The activities delegated to the Support Care Associate are determined by the policies and procedures of the Nursing Department and are in accordance with the requirements of the patient care unit
Performs related duties, as required.

EXPERIENCE AND QUALIFICATIONS
High School Diploma or equivalent, required.
Ability to communicate effectively, required.
