Job Category Management
Manages the daily operations of Case Management Department and Case Management staff. Ensures delivery of care, productivity and timeliness. Ensures staff is informed of changes impacting their work by communicating updates, policy revision, performance outcomes and significant events. Evaluates Case Managers practice for quality, timelines and process consistency. Assumes all responsibilities in absence of Director of Case Management.
Participates in the evaluation of Case Management Department activities.
Selects, develops, manages and evaluates direct reports; and oversees the selection, development, management and evaluation of indirect reports.
Coordinates staff orientation and training programs. Documents and assesses effectiveness of staff training.
Ensures staff provides case management within scope of practice in compliance with Department protocols.
Coordinates staff in-services with key internal/external customers to resolve beneficiary care issues and improve access.
Monitors individual staff caseloads and beneficiary treatment plans to ensure effective delivery of care, productivity and timeliness.
Develops staff performance evaluations in collaboration with Director of Case Management.
Develops Case Management reports to report Departmental outcomes, trends and issues.
Collaborates with medical providers, the Director and NSUH Nursing Leadership, on program interpretation and guidance.
Revises operational processes, polices and procedures to ensure overall program effectiveness.
Performs related duties, as required.
• Bachelor's Degree in Nursing, required. Master's Degree, preferred.
• Current license to practice as a Registered Professional Nurse in New York State.
• Certified Case Manager (CCM), Nurse Case Manager (RN, CM), Case Manager, Certified (CMC) or Certified Professional in Health Care Quality (CPHCQ), preferred.
• Minimum of five (5) years administrative/supervisory case management experience in an acute care environment, required.
